If you are in need of accommodations or services from our office, please complete the following form. The more information you provide, the more we will be able to serve you. It is imperative that you check your phone and your MCC-issued email address as those are the two primary ways that our office will communicate with you initially.

By submitting this application, you agree to the following:

1. I am responsible for initiating an application with Access and Disability Services (ADS) to request accommodations and/or services. These requests will remain pending until ADS notifies me that they have been approved. ADS may need additional information or documentation from me before approving my requests.

2. To use my approved accommodations, I must: (a) send my accommodations to my instructors through the ADS student portal each semester; and (b) communicate with my instructors about the accommodations that I intend to use. If I choose not to request or use my approved accommodations, they cannot be granted retroactively.

3. I am responsible for informing ADS staff and my instructors of my needs so that they can connect me to available resources.

4. I am responsible for regularly checking and responding to email and phone messages from ADS. For MCC students, ADS will contact you at your MCC-issued email address.
